Maxillo 3 Lacs’ Oral & Maxillofacial Surgeons, Dr. Michel El-Hakim, Dr. Nicholas Makhoul and Dr. Gigliotti, practice a full scope of Oral and Maxillofacial Surgeries with expertise ranging from dental extraction, dental implants, to corrective jaw surgery and complex facial reconstructions. M3L’s surgical team can also diagnose and treat facial pain, facial injuries, and perform a full range of corrective and esthetic procedures. We are proud to deliver the finest surgical care at the most reasonable cost to our patients therefore payment is due at the time service is rendered unless other arrangements have been made in advance. We will provide you with the necessary information in order for you to claim your expense to your private insurer.

Dental insurance

Some dental treatments may be covered by your insurance plan, while we do not submit insurance claims directly, we will be happy to provide you with the necessary forms for you to submit your claim yourself. If your insurance company has any questions or requires additional information our team will assist you in order to complete your claim.

Most insurance companies will respond within four to six weeks. Please call our office if you have questions and please remember you are fully responsible for all fees charged by M3L’s office regardless of your insurance coverage.

We will prepare a detailed treatment plan with cost information during your initial consultation and take the time to answer any questions you may have about insurance and reimbursable costs. Our financial policy is designed with our patients’ needs in mind and we accept several third-party financing options.
